In-home aged care services are available to individuals aged over 65 with health conditions who are Australian residents. Eligibility for the Commonwealth Home Support Program (CHSP) or a Home Care Package (HCP) is determined by an assessment team through My Aged Care.

RAS Assessor Home Visit

Following your initial contact, a Regional Assessment Service (RAS) assessor will conduct a home visit to assess your care needs.

CHSP Services Approval

If suitable, you may receive approval for services under the Commonwealth Home Support Program (CHSP).

Contact Service Providers

Once approved, you can contact 1st Care Community as your service provider to commence the delivery of care and support services.

If the CHSP is unable to fully meet your needs, don’t worry. Our team can guide you through the process of obtaining further assistance:

Referral to ACAT Assessment Team

If necessary, your doctor or the RAS assessor may refer you to the Aged Care Assessment Team (ACAT) for a more comprehensive assessment, which can result in Home Care Package approval or residential care approval.
